You have to print AquaSource distributor form in two copies for AquaSource office, fill in "Distributors information", "Distributor's delivery address" and "Applicant's signature - your signature.

If you pay by bank card (which is the simplest way), please, complete data on bank card and sign the part of "signature".

You can scan the completed and signed form and send it by email, fax or by post to the AquaSource office at the address listed at the beginning of the form.

A document with the original signature (Distributor Agreement Form) must be sent by post at the AquaSource office in two copies (2 originals - with your signature for the AquaSource office) and one original - with your signature to keep for you.
